Cancer Docs Often Deal With Own Grief, Doubts When Patients Die

WEDNESDAY, May 23 (HealthDay News) -- Some cancer doctors may build up emotional walls -- distancing themselves from the patients they can't save -- to avoid grief, sadness and even despair, new research shows.

In a profession where death and dying "are part and parcel of the work," study author Leeat Granek said grieving is mixed with "feelings of self-doubt, failure and powerlessness that come from the idea that doctors are responsible for their patients' lives and for making their treatment decisions."

Twenty oncologists at three adult cancer centers in Ontario described how they dealt -- or didn't deal -- with grief, and its effect on their professional practice and personal lives. The report was published online May 21 in the Archives of Internal Medicine.

"The issue with doing oncology is that you walk a very fine line," one doctor said. "If you get too involved with your patients you can't function because it's too much emotional load to bear, and if you get too distant from your patients then I don't think you're being a very good physician, because people pick up on that."

"Sometimes the grief comes home with the oncologist," said Granek, a postdoctoral fellow at the Hospital for Sick Children in Toronto. Although some doctors "compartmentalized" in order to function, others had difficulty sleeping or enjoying time with their family.

But there were positive reactions, too. Some physicians found they had a better perspective on life from frequent exposure to patient loss. And some felt motivated to give better care.

The study referred to the physician's burden of "holding hard knowledge."

"Sometimes I'll take a chart and I'll look at the imaging, and everything's worse and the numbers are worse," a doctor said, "and I have to drag myself into the patient's room and figure out what can I offer them that's hopeful and positive. It's tough."

Several oncologists said they cried on the way home in their cars. But feelings were kept private or submerged.

"Losing any patient is difficult," said Dr. Len Lichtenfeld, deputy chief medical officer at the American Cancer Society. "But there is no time to grieve. You have a moment or two to reflect and then you're on to the next patient who needs your help."

Granek said that "even just acknowledging that grief over patient loss exists and that it's part of the profession would be healthier than what is happening now. There is no acknowledgement at all, and there's denial."

Other patients might be affected in the aftermath of a loss, some respondents suggested.

"Maybe I got that case after someone had just died and I was in a more aggressive mode," one physician said. "Or, maybe I undertreated someone because I just saw a patient with terrible toxicity."

Doctors talked about being distracted, less focused. When patients were dying, some physically distanced themselves, avoiding the hospital and bedside.

Patients and family members can sense this distance. Lichtenfeld told of a bereaved husband who confided in him after his wife had died of breast cancer.

Caring professionals "would spend time with him, talk to him, look at him, hold his hand, ask how he was feeling," Lichtenfeld said. But others "would not engage in those behaviors -- they would look the other way, they would not look him in the eye, they would rely on 'the data, the data, the data,'" in conversations.

"If you're troubled about the way the physician or members of that team are relating to you," Lichtenfeld recommended "bringing it up as part of the conversation."

An accompanying journal commentary described how one institution is dealing with the situation. Since 2008, the University of Rochester Medical Center in Rochester, N.Y. has held staff support meetings -- mandatory for oncology fellows -- where practitioners are encouraged to discuss their experiences with patient loss and grief.

"Feelings of frustration, anger, loss, isolation and insecurity often emerge in a setting that is nonjudgmental and supportive," wrote oncologists Dr. Michelle Shayne and Dr. Timothy Quill. "At the end of each one-hour session, a moment of silence is observed in remembrance of patients who have recently died, and the opportunity to remember and honor a patient who has died by saying his or her first name is offered."

Oncologists "are working very hard and doing this phenomenal job with very large numbers of patients," Granek said, "and they could use a little bit of support with this piece."

2010 census missed more than 1.5M minorities

WASHINGTON (AP) ? The 2010 census missed more than 1.5 million minorities after struggling to count black Americans, Hispanics, renters and young men, but was mostly accurate, the government said Tuesday.

The Census Bureau released an extensive assessment of its high-stakes, once-a-decade headcount of the U.S. population. Based on a sample survey, the government analysis has been a source of political controversy in the past over whether to "statistically adjust" census results to correct for undercounts, which usually involve minorities who tend to vote Democratic.

The findings show the 2010 census over-counted the total U.S. population by 36,000 people, or 0.01 percent, due mostly to duplicate counts of affluent whites owning multiple homes. That is an improvement from a census over-count of 0.5 percent in 2000.

However, the census missed about 2.1 percent of black Americans and 1.5 percent of Hispanics, together accounting for some 1.5 million people. The percentages are statistically comparable to 2000, despite an aggressive advertising and minority outreach effort in 2010 that pushed total census costs to an unprecedented $15 billion.

Also undercounted were about 5 percent of American Indians living on reservations and nearly 2 percent of minorities who marked themselves as "some other race."

"While the overall coverage of the census was exemplary, the traditional hard-to-count groups, like renters, were counted less well," Census Bureau director Robert Groves said. "Because ethnic and racial minorities disproportionately live in hard-to-count circumstances, they too were undercounted relative to the majority population."

"Our belief is that without our outreach, our numbers would have been much, much worse," he added.

The South, led by the District of Columbia, Texas, Georgia, Virginia, North Carolina and Florida, was more likely to have people who were missed. The Midwest and Northeast as a whole posted small over-counts.

The findings come after more than 100 cities including New York challenged the official 2010 results as too low.

The Census Bureau, which recently rejected New York's request to revise the city's count, says the latest analysis will not affect the government's official U.S. population tally of 308.7 million but it will be used to improve the 2020 count. Nor will the analysis affect how the federal government distributes more than $400 billion to states for roads, schools and social programs.

"We remain deeply troubled by the persistent and disproportionate undercount of our most vulnerable citizens ? people of color, very young children and low-income Americans," said Marc Morial, president of the National Urban League and chairman of the Census Bureau's 2010 Census Advisory Committee. "At a minimum, the census should have the ability to make an adjustment in the official count to ensure that these individuals enjoy the political representation and fiscal resources to which they are entitled."

The Supreme Court ruled in 1999 that federal law barred the use of sample surveys to adjust census results for purposes of allocating House seats; it left the door open to adjustments for other uses such as congressional redistricting or distribution of federal funds. Shortly after taking office in mid-2009, Groves ruled out statistical adjustments in 2010 for redistricting, citing a lack of preparation time.

On Tuesday, the Census Bureau noted how its efforts to count U.S. residents have improved over time. An undercount of the total U.S. population reached as high as 5.4 percent in 1940, the first time the accuracy of a census was formally measured, and then gradually decreased before an over-count was posted in 2000. American blacks are still the most likely to be missed; their undercounts have improved from a high of 8.4 percent in 1940 but at a slower pace than that of whites.

The government takes a census survey every 10 years. The bureau sends census takers and questionnaires to every U.S. household, though not everyone responds. In 2010, the government faced special challenges of counting transient families displaced by widespread mortgage foreclosures, non-English speaking immigrants fearful of enforcement raids and distrustful Americans opposed to government surveys.

Among the findings:

?Renters were undercounted by 1.1 percent, while homeowners were over-counted by 0.6 percent.

?Broken down by age, men 18 to 29 and 30 to 49 were more likely to be missed in 2010 than other age groups, while women 30 to 49 were over-counted; that is a pattern consistent with 2000. Adults 50 and older had over-counts of their population, while some young children ages 4 and under were missed.

?The District of Columbia had the highest shares of people who were missed, at 2.2 percent. West Virginia had the highest over-count of its population, at 1.4 percent.

Democrats and Republicans for years have disagreed on whether the census should be based on a strict head count or cross-checked against a "statistical adjustment" to include hard-to-track people, particularly minorities, who might have been missed.

While statistical adjustment was ruled out in 2010, the issue could re-emerge for the next census in 2020 as the agency struggles to find ways to rein in ballooning costs to its traditional paper mailing operation while striving to reduce a persistent undercount among minorities. Groves is leaving his post in August to become provost at Georgetown University, and in an election year President Barack Obama has not yet nominated his successor.

"Given the difficult economic climate leading up to the 2010 census ? with Latinos hit particularly hard by foreclosures, job-loss and other issues that can hinder census participation ? the results of this census could easily have been far worse," the National Association of Latino Elected and Appointed Officials said in a statement. "As the U.S. population becomes more diverse, it is increasingly important for the country to invest in funding that will allow the Census Bureau to address the undercount challenges."

And, judging by how comfortably the M60 fits in the palm of your hand, there will be plenty of prolonged use, both on and off the gaming grid.

Five smooth plastic glide pads on the Vengeance M60's underside effectively minimize friction during movement, resulting in a smooth-as-butter operation, even on plastic or fabric surfaces. And while the M60 feels appreciably sturdy right out of the box, three removable weights on the underside allow you to adjust the weight depending on your preference. Still, there's no denying that the M60 feels as great as it looks.

The Vengeance M60 is a wired mouse that connects via USB 3.0. There's no wireless option, which will undoubtedly turn off those who prefer the clutter-free convenience and freedom that Bluetooth or 2.4GHz wireless mice offer. I tested the Vengeance M60 with a few rounds of Team Fortress 2, and was pleasantly surprised by the results. The bodies of four people missing after a boat crash on the Mississippi River in Iowa were found Sunday within 100 yards of the crash site, according to the Iowa Department of Natural Resources. None was wearing a life vest.

DNR officer Paul Kay said three men and a woman, all believed to be in their 20s, were killed. Their names have not been released, pending notification of their families.

Along with the four killed, eight people were injured when two johnboats ? flat-bottomed boats often made of aluminum ? crashed in the O'Connell Slough area of the river, where the water is 10 to 12 feet deep, before 2 a.m. Saturday. One of the boats was carrying 11 people; the other had a single passenger.

Authorities scoured the river near Burlington for signs of the missing boaters Saturday before calling off the search after dark. Efforts resumed early Sunday morning.

DNR spokesman Kevin Baskins said eight people were pulled from the water soon after the crash. At least one person suffered serious injuries and was flown to an Iowa City hospital, he said. The others were taken to a Burlington hospital. Some had since been released, Baskins said Sunday, although he didn't have an exact count.

Baskins said authorities are still sorting out the cause of the crash. No charges had been filed as of Sunday.

"That's going to take a while," he said of the investigation.

Burlington is in southeast Iowa on the Illinois border, about 200 miles north of St. Louis on the river.

(AP) ? A Myanmar negotiator says ethnic Shan rebels have agreed in a second round of talks to end fighting with army troops in the country's east, the latest reported deal between the new reformist government and various ethnic separatist movements.

Negotiator Aung Min, who is also the country's railway minister, said he discussed enforcing a December cease-fire and eradicating drugs in eastern Myanmar in talks with Yawd Serk, leader of the Shan State Army (South).

He told reporters Saturday night after the talks in the Shan State capital, Kengtung, that recent clashes were a result of misunderstanding over the firing of warning shots.

"After detailed negotiations today, there will be no more fighting," Aung Min said.

Myanmar President Thein Sein's government has reached cease-fires with several ethnic rebel groups, but fierce fighting continues with the Kachin minority in northern Myanmar, with whom peace talks have so far failed.

The state-run New Light of Myanmar newspaper on Sunday reported that the Kachin guerrillas had blown up several electricity pylons on Friday. "So, electric power supply will be reduced for a while across the nation as personnel concerned are trying to repair the towers as possibly quick as they can," it said.

Much of Myanmar has long suffered from frequent power shortages, due to resource mismanagement that predates the fighting.

The state press has recently been highlighting such attacks in an apparent propaganda ploy to pin the blame on the Kachin for the continuing fighting. Thein Sein late last year issued an order for the army to cease its offensive against the Kachin, but it has not been effective.

Since taking office last year as a military-backed but elected president, Thein Sein has moved to roll back many of the repressive actions of the military regimes that preceded him.

His efforts have included reconciliation with opposition leader Aung San Suu Kyi and her pro-democracy movement and also the long-running problem of ethnic rebellions.

Myanmar's ethnic minorities, who are clustered mostly in border areas, have long sought greater autonomy.
